OF PROGRAM: Adult Evaluation and
Review Services (AERS) initially was known as Geriatric Evaluation
and Review Services when the program originated in 1974. In Allegany
County AERS also serves as the lead agency for the Senior Care Program
and the Family Caregiver Support Program. The AERS programs collaborate
with various community and state agencies including Western Maryland
Health System, Allegany County Department of Social Services, Allegany
County Housing Authority, Human Resource Development Commission, Area
Agency on Aging, area nursing homes and adult day care facilities.
The AERS, Senior Care, and Family Caregiver Support Programs conduct
in-home and on-site visits to evaluate individuals and refer them
for services they may be eligible to receive.
Senior Care provides in-home services and is able to purchase
limited supplies for eligible clients.
Family Caregiver employs a full-time Certified Nursing Assistant
who is able to help clients in their homes with personal care several
Both Senior Care and Family Caregiver have an ongoing waiting list
due to the limited supply of funding.
AERS provides no-cost evaluations for aged and functionally disabled adults who need long term care and are at risk of institutionalization.
Senior Care provides services to clients who are age 65+, who
meet income and asset guidelines, and who are severely or moderately
disabled based on the AERS Assessment. Income guidelines are adjusted
yearly, but the Assets must be less than $11,000 for an individual
and less than $14,000 for a couple.
Family Caregiver clients must be age 60 and older, with no specific
income or asset guidelines. The clients must have a caregiver who
is in need of respite in order to be eligible.
TO OBTAIN SERVICES: Referrals are taken from other agencies, the
client, family, or any concerned individuals. Usually an appointment
is made by phone and a nurse or social worker will conduct an in-home
or on-site visit to assess the client's needs.
