Birth certificates are available at the Allegany County Health Department for births from 1940 to the present, for a fee of $20* (effective July 1, 2016). (Cash, local checks, Visa, MasterCard, and Discover card accepted.) Certified birth certificates can be acquired within a half hour of coming to the health department as long as all guidelines listed on this web page are met.
Birth certificates for years prior to 1940 must be requested from the Division of Vital Records, 6550 Reisterstown Road, Baltimore, MD 21215. The fee is $10.00 when applying to the Baltimore office.
Birth certificates may only be obtained by those persons who are named on the certificate (i.e., mother, father, or the individual who is the subject of the birth certificate). Proper government-issued photo identification is required. See the list at the bottom of this page for accepted forms of ID.
All birth certificate requests involving court orders, powers of attorney, or notarized letters must be done through the Baltimore office.
IMPORTANT NOTE: If you are planning to travel outside the United States, you must obtain a certified copy (with a raised seal) of your Maryland birth certificate. The Allegany County Health Department issues certified copies. You may not substitute a photocopy of the certificate, a Birth Registration Notice, a Statement of Age card, or a hospital-generated birth record. * NOTE: There is no fee for a copy of a birth certificate of a current or former armed forces member when the record is requested by that member. Specific documentation is necesary to receive a copy free of charge for the military. Please call the number below for more information.
Death certificates are issued to funeral directors for deaths that occurred after January 1, 2015. Deaths that occurred prior to January 1, 2015 can only be obtained through the Baltimore office of vital records.
Certified death certificates can be issued to immediate family members for 30 days from the date of death if they are listed as the informant on the certificate. They must present a valid driver's license, MVA-issued photo ID card, or United States Passport. All identification must have an issue and expiration date.
The cost of a death certificate is $18.00 for the first copy and $20.00 for each additional copy.
After 30 days, a death certificate requested by a family member must be requested from the Division of Vital Records, 6764 B Reisterstown Road, Baltimore, MD 21215. The fee is $10.00 for the first copy and $12.00 for each additional copy in the same transaction when applying to the Baltimore office.

Acceptable Forms of Identification
Proper government-issued identification is required to obtain vital records. No side view photo IDs are accepted. IDs that are expired, cracked, chipped, broken, frayed, faded, or peeling will not be accepted. We reserve the right to refuse any ID we deem questionable.
Acceptable forms of ID include the following:
- Valid Driver's License
- United States Passport
- Department of Motor Vehicles Photo ID Card
- Military Photo ID
IDs must include an issue date and an expiration date. If the ID does not include a full middle name, an additional form of ID will be required, such as a Social Security Card.
